Advanced Sperm Selection Alongside ICSI

Physiological intracytoplasmic sperm injection, or PICSI, is an additional sperm-selection technique used during selected ICSI cycles. It helps the embryology team identify sperm that bind to hyaluronan, a substance naturally present around the egg.

How Does PICSI Work?

On the day of egg collection, the semen sample is prepared in the laboratory. Rather than selecting sperm only by appearance and movement, the embryologist may use a specialised PICSI dish to identify sperm that bind to hyaluronan. Selected sperm are then used for ICSI, where one sperm is injected into each mature egg.

When Might PICSI Be Considered?

PICSI is not necessary for every ICSI cycle. It may be discussed in selected situations, such as previous fertilisation or embryo-development concerns, repeated IVF/ICSI failure, or where the clinical and embryology teams believe an additional sperm-selection method may be useful.

What to Expect

PICSI is performed in the embryology laboratory as part of an ICSI cycle; it does not add a separate procedure for the patient. The rest of the IVF/ICSI process—stimulation, egg retrieval, embryo culture, transfer, and pregnancy testing—continues as planned.
